DUI Evaluation

Anyone convicted of a DUI in the state of IL needs to complete an alcohol/drug evaluation. If you are currently attending court and need to be evaluated please refer to the following:

For Cook County DUI’s: Social Services Dept.

For Lake County DUI’s: NICASA

For DuPage County DUI’s: DuPage Adult Monitoring Dept.

Please have the following for your appointment: ID, Copy of Driving Abstract (can be purchased online cyberdriveillinois.com), Documents from date of arrest that include a BAC (Summary Suspension), and/or any other applicable documents pertaining to the case. To qualify for indigent fee please talk to staff for more information. Available in Polish & Spanish (Polish Tuesday & Thursday. Spanish only Saturday)
